One original and one copy of the <br />following documents are submitted: <br />1. Revised page 3 of the Construction Materials Limited Impact (I 10) Operation <br />Reclamation Permit Application Form. Specifically, item 10 is revised to complete the <br />quarry location information. <br />2. Revised Exhibit G and new Exhibit G-1 (tax receipt). <br />3. Revised Exhibit I, including Attachments Exhibit I -I, Exhibit I-2, and Exhibit I-3. <br />Exhibit 1-1 is unaltered from the originally submitted application. Exhibit I-2 is a new <br />exhibit and is an affidavit indicating the date on which the revised application was placed <br />with the Jackson County Clerk and Recorder.
