March, 2012 and Trautner Geotech, LLC; <br />• Bench Geotechnical Investigation –(Attachment4); <br />• Portal Designed; <br />• Removal of collapse material from the access and mill portals; (Bench stability report <br />prepared by R Squared, Inc. was previously submitted) Portal initial work plan was <br />submitted on March 2 [EnviroGroup-Access Portal Collapse Repair and Equipment and <br />Stope Rock Removal Plan], Adequacy Review, March 8, 2012 and approved by CDRMS <br />on March 12th, 2012 ; <br />• Removal of buried equipment and collapsed debris from the access and mill portals; <br />[Collapsed debris (photos 1 and 2) was stock piled adjacent to May 2 2016 portal];Used <br />shotcrete (reinforcing medium), split set bolts to secure 4X4 welded wire fabric (See <br />Photo); <br />• Geotechnical studies to specify split set spacing pattern; (See photo); Portal <br />stabilization (Attachment 5-Franklin (February 22, 2012). Trautner observed steel set <br />construction/installation Attachment 6; <br />• Removal and placement of damaged steel set(s) and protective canopy inside the <br />access portal; [Buried equipment was removed by Wildcat Mining Corporation and <br />the access portal was designed ( Figure 2) and replaced by Melcher Brothers]; Adit <br />Stabilization – Melcher Brothers-Observed adit repair and assessed highwall stability <br />features to verify construction was in accordance with the construction drawings and <br />specifications. (See Figure 1-As-built Topographic Map); <br />• Site grading and gravity wall construction (under Trautner Geot supervision); <br />• Installed steel sets (Attachment 7); <br />• Installed bench and upper terrace stormwater control Best Management Practices; <br />• Installed headwall and bench stability survey monuments; (Mountain Man,2016); <br />
