<br /> <br />Wildcat Mining Corporation <br />May Day and Idaho Mine and Mill Application <br />112(d) Permit – Technical Revision No. 1 <br />May Day and Idaho Mine <br />M-1981-185 <br />Revised June 30th, 2016 <br />3 <br />Technical Revision-TR-5 <br />Removal of mine debris from Little Deadwood Gulch in the Chief Portals Area, installation <br />of a culvert within LDG Channel,-Stabilization of the Highwall and access Road Above the <br />Chief Portal, and Repair of the Emergency Manway <br />May Day Idaho Mine Complex <br />M-1981-185 <br />Wildcat Mining Corporation <br />June 30, 2016 <br /> <br />1) Introduction <br />On March 3rd 2016, the Colorado Mined and Reclamation Board (CMLRB) issued <br />Wildcat Mining Corporation (Wildcat) an order requiring the approved Chief portal and <br />Little Deadwood Gulch Technical Revision (TR-5) to be completed no later than July 1, <br />2016. Because of the delays in obtaining Army Corp permits, the deadlines to complete <br />Phase 1 (Attachment 1-Little Deadwood Gulch Culvert installation) the has been <br />extended to October 15, 2016 whereas Phase 2 Chief Portal and Road Stabilization <br />(Chief) construction must be completed on or before July 1, 2016. <br /> <br />The following is the Chief Portal stabilization and maintenance completion report and a <br />supplemental Little Deadwood Gulch (LDG) drainage restoration report will be submitted <br />when drainage construction and restoration activities are completed. <br /> <br />The technical scope of TR-5 includes the following activities: <br /> <br />Phase 1 –Little Deadwood Gulch Channel Restoration Scope of Work (Revised Completion <br />Schedule) <br /> <br />• Restore Little Deadwood Gulch (LDG) in accordance with CDRMS approved drainage <br />restoration plan. The LDG (Phase 1) drainage restoration plan must be completed on <br />or before October 15, 2016. <br /> <br />Phase 2 –Chief Portal Stabilization Scope of Work <br /> <br />• Loose Fill and debris should be removed from the highwall area and a buttress should <br />be constructed to establish a pad on which the portal improvements can be supported. <br /> <br />• The finished slope of the highwall shall not exceed 1.5:1. <br /> <br />• Fill material should be free of organic material with the largest particle size less than 6 <br />inches, moisture conditioned and compacted. <br />
