5 <br /> June 2016 all reclaimed production well pads were spot sprayed. Back <br /> pack spraying equipment is now used to access all of the reclaimed pads <br /> and the individual thistle plants are targeted. Large mature thistle are <br /> removed and disposed of as spraying is not effective in control at this <br /> stage. <br /> 14. Is adequate topsoil reserved for reclamation, based on your approved YES NO N/A <br /> permit? If"No", please explain: <br /> 15. Is the reserved topsoil vegetated/stabilized in accordance with Rule YES NO N/A <br /> 3.1.9(1)? If"No", please explain: <br /> 16. If mining has exposed groundwater, is the site in compliance with the <br /> approved mining plan and Office of the State Engineer (Well Permit, YES NO N/A <br /> S.W.S.P., and/or Permanent Augmentation Plan)? <br /> 17. We hold a bond with BLM for just under$5 million and an <br /> administrative bond with DRMS for just over$236,000. All of the YES NO N/A <br /> production wells are plugged and abandoned so the EPA released their <br /> bond under the UIC program in Dec. 2015. <br /> 19. Is your basis for legal right to enter still valid? YES NO <br /> 20. Does your permit require you to submit monitoring information <br /> annually? If"Yes", please attach the required monitoring results to this <br /> Annual Report. (Ground water monitoring reports were submitted <br /> quarterly (for 60 quarters) until all production wells were plugged and YES I NO N/A <br /> abandoned in 2015. Nearly all of the ground water monitoring wells are <br /> presently plugged and abandoned as of this report. Reclamation on these <br /> pads and roads will continue into fall 2016.) <br /> 21. As required by rule, attach a map to this report that accurately depicts CURRENT MAP <br /> the permit boundary, current affected area boundary and location of the ATTACHED <br /> acreages specified in items 7— 12 and 14. <br /> Division records indicate the following permittee contact information.
