PERMIT#:M-2006-039 <br /> INSPECTOR'S INITIALS: ACY <br /> INSPECTION DATE: May 16,2016 <br /> OBSERVATIONS <br /> This inspection was conducted by the Colorado Division of Reclamation, Mining and Safety (Division). This <br /> inspection was requested by the operator to follow up on potential issues identified in the April 13, 2016 <br /> inspection. The North Clear Creek Construction Materials Mine is a 110c permitted site that includes a total of <br /> 8 permitted acres. This site is located approximately 17 miles northwest of Debeque, Colorado and is accessed <br /> from Garfield County Road 211 (North Clear Creek Rd). Richard Grubbs and Mark Sage represented Chevron <br /> USA, Inc. and accompanied Amy Yeldell and Russ Means of the Division on the inspection. <br /> Following the April 13, 2016 staff noticed that maps submitted in the 2015 annual report did not coincide with <br /> the boundary delineated in the permit application. Upon further investigation staff digitized the 2015 annual <br /> report boundary and it appeared to be approximately 11 acres. Based on the google earth imagery the land <br /> appeared to have been disturbed at some point and has since been reclaimed. Based on this uncertainty of <br /> affected lands the Division required the operator to survey the current disturbance to clarify the total affected <br /> acreage to date. The operator shall provide the Division with documentation of their findings by July 19, 2016. <br /> The purpose of this inspection was to delineate the boundary of affected land. This site has had previous <br /> disturbances outside of the permit boundary. According to records the purpose of these disturbances are <br /> unclear since Chevron has had a long history of oil shale and natural gas production in this area in addition to <br /> the borrow material. <br /> Land that has been affected and that is still un-reclaimed is approximately 5.5 acres. The remaining areas in <br /> question have been successfully reclaimed and would be releasable per DRMS standards. Reclaimed areas <br /> have made excellent use of pocketing to both help stabilize the soil and create micro ecosystems where <br /> vegetation thrives. Areas with older reclamation have establishment of shrubby species in addition to native <br /> grasses. <br /> Based on this site inspection the operator shall provide the Division with updated maps of currently affected <br /> lands. As well as depict an 8 acre boundary to be used for future mining. These revised maps shall include both <br /> mining and reclamation maps with all necessary information per Rule 6.3.5. They shall conform to Rule <br /> 6.2.1(2). These updated maps shall be submitted in the form of a technical revision (TR) with the required fee <br /> of$216 by July 19, 2016. <br /> Addition items mentioned in the previous inspection report are as follows. Permit boundary marker were <br /> difficult to distinguish at a distance. Since the April inspection the markers have been covered with reflective <br /> tape and are now easily visible. The operator is remained to make sure any new markers are also easily visible. <br /> The operation will need to be approved for inert fill prior to accepting off-site material. The steps for which <br /> were outlined in the previous inspection report and will require a technical revision. This site is listed as an <br /> active operator however it was indicated that this site is active annually but less than 180 days a year. Please <br /> request to be an intermittent operator in the form of a technical revision. <br /> The Division would like to remind the operator that several minor changes can be submitted under one <br /> technical revision. The operator is also encouraged to review their reclamation and mining plans to make any <br /> necessary changes. <br />
