North Clear Creek Construction Materials Mine <br /> Response/TR/Fee for P-2006-039 Inspection Report Dated May 16, 2016 <br /> Dear Ms. Yeldell: <br /> Chevron (OWNER/OPERATOR) is in receipt of the inspection report from your site visit on April 13, 2016 and <br /> follow-up site visit on May 16, 2016. Attached is the Technical Review JR) requested by the inspection <br /> correspondence. <br /> The April 13, 2016 inspection highlighted these items requiring response as a TR: <br /> 1. Survey of current disturbance to clarify the total affected acreage to date <br /> 2. Improve visibility of Permitted Boundary markers <br /> 3. TR to address imported inert fill that was observed but not allowed under the current permit <br /> 4. Request to make status Intermittent Operation that is active annually but less than 180 days a year <br /> 5. Provide fee of$216 for Technical Review JR) <br /> Chevron provides the following discussion and supporting documentation in response to each item. <br /> ITEM 1 — Following the April 13, 2016 inspection and report, CDRMS revisited the site on May 16, 2016 to <br /> provide delineation of boundary of affected land, prior to the Operator completing the requested survey. The <br /> requested survey was completed by Uintah Engineering and Land Surveying and depicts the active mine <br /> disturbed area shaded in light green is 6.49 acres including working areas and topsoil stockpile. The <br /> revegetated mine area extending to the boundary on the North and East side of the mine is shaded in a <br /> darker green and is 1.51 acres. The total mine boundary is 8 acres. The 3.44 acre area shaded in red south of <br /> the south mine boundary shows previously disturbed area not associated with mining activities. SEE <br /> ATTAHCED UINTAH ENGINEERING MINING MAP and RECLAMATION MAP. <br /> ITEM 2 — As shown in Figure 1 and Figure 2 below, high visible markers have been installed along the <br /> boundary of the mine providing delineation of the mine operation. <br />
