Box 806 <br />Salt Lake City, UT 84110 <br />RE: Last Call for Delinquent Annual Report and Map, Camp Bird Mine, Permit No. M-1982-090 <br />Dear Mr. Butters: <br />Pursuant to C.R.S. 34-32-116(3) and (4), and the conditions of Permit No. M-1982-090, you are <br />required to submit $633 in annual fees and an annual report and map by the anniversary date of <br />the Permit, by December 71l' of each year. As of the date of this correspondence, the Operator is <br />current regarding the payment of the annual fees. However, the Division has not received the <br />annual reports and maps for two consecutive years, due December 7, 2014 and December 7, 2015, <br />Given the unique circumstances of this specific site, the Division has granted a limited grace period <br />for receipt of the delinquent annual reports and maps, now due June 20, 2016.
