Box 952 <br /> 1200 Hazel Street <br /> Leadville, CO 80461 <br /> Phone: 719-486-0259 <br /> Fax: 719-486-8479 <br /> June 29, 2016 <br /> Annual Report <br /> Division of Reclamation, Mining & Safety <br /> Department of Natural Resources <br /> 1313 Sherman Street, Room 215 <br /> Re: Permit No. M-1977-459 <br /> Lake County Gravel Pit <br /> Anniversary Date: July 8, 2016 <br /> Gentlemen; <br /> Enclosed, please find the payment of$791.00 for the annual fee for the above referenced <br /> permit. <br /> The activity in the calendar year 2015 consisted of mining and crushing approximately 20,000 <br /> tons of road base material during the time period January 1, 2015 to December 31, 2015. The <br /> material mined consisted of taking down material on existing disturbed areas as shown on <br /> enclosed map and cutting down the slope on the south east side of the pit. No new areas of <br /> disturbance occurred during this time period. <br /> Reclamation for the year 2015 consisted of stockpiling existing top soil from the pit area to the <br /> east side of the pit. Also in 2015, we have continued our program of stockpiling top soil from <br /> outside sources, such as new road construction around the County in anticipation of future <br /> reclamation work. <br /> During the year 2015, since there were not any new areas of disturbance Lake County <br /> continued to accumulate top soil from outside sources. It is expected that there will not be any <br /> new disturbances 2015. All mining activities will be within previously disturbed areas of the <br /> existing pit. Production for the calendar year January 1, 2016 through December 31, 2016 will <br /> be between 20,000 and 25,000 tons of road base material. <br /> As in the past, Lake County also uses this mine site and crushing equipment to recycle asphalt <br /> that has been removed from County and City roads. This practice while not a mining activity has <br /> been in effect for 10 plus years.
