Box 191 <br />Victor, CO 80860 <br />Re: Project, Permit No. M-1980-244; <br />Incomplete Technical Revision (TR-78) Request <br />Dear Mr. Henris: <br />On July 1, 2016 the Division of Reclamation, Mining and Safety (Division) received a request for a <br />Technical Revision. There were some notable discrepancies between the information provided on the <br />Division’s cover sheet and that stated in CC&V’s cover letter. The cover letter indicated the only <br />proposed revision to the permit was for “Power and light installation at the grizzly/conveyor system”; <br />whereas the cover letter indicated updated/revised specifications for the Squaw Gulch Valley Leach <br />Facility were to also be considered in this revision. The letter further indicated the revised <br />specifications were included with the submittal, but the Division did not receive the referenced <br />specifications. Finally, the cover letter identified the submittal as TR#79. The Division’s records <br />indicate this submittal will be TR-78. <br />On July 6, 2016, I contacted Tamara Baker (CC&V) to help clarify the intent of this TR request. Ms. <br />Baker indicated the intent was to include the updated specification changes. The Division requires <br />the following for this TR submittal to be considered complete for the purpose of filing: <br />1.A revised Division “Request for Technical Revision (TR) Cover Sheet” that includes both the <br />power and light installation, and the revised specifications in the description (Note: include a <br />note on the fee schedule at the bottom of the form that the fee was already submitted and attach <br />a copy of the receipt included with this letter); <br />2.Revised Newmont cover letter changing the TR # to 78 and stating the cover letter was revised <br />to transmit the updated specifications. <br />3.Please also include the two drawings (annotated satellite image and power diagram) previously <br />submitted. It would be helpful if these drawings were also numbered and labeled (e.g., <br />Drawing 1. Location of buried powerline.)
