Skies were clear and a slight breeze blew. Temperatures hovered at 85 degrees F.

Records were inspected at Goff Engineering the previous morning, Tuesday, 21 June 2016 from 10:30 to 11:30 A.M.
Maintenance items of previous inspection had been addressed. Gullies identified in the March inspection along Ewing Mesa
Road had been stabelized, see before and after photos 5A-5D below.

PERMITTING UPDATE
A midterm review of the permit was conducted by DRMS and submitted to the operator in January of 2016. DRMS
received a response in in February of 2016 and is awaiting an application for a minor revision from the operators
consulstant to address topics resulting from the review.

JUNE INSPECTION:

SIGNS AND MARKERS Rule 4.02

 Topsoil stockpiles observed were marked with identifying signs.
 Mine identification signs at gate were installed as required and displayed correct information.
 Permit boundary signs observed appeared to be ade quate, and in good repair with the exception of:
1. (Outstanding from March inspection) Map 5 -7 appears to have several discrepancies
related to the permit boundary and the coal disturbed area If there has been some
agreement made with the Ewing Mesa Gravel Pit as to their boundary and the Carbon
Junction permit boundary please provide this documentation or reconcile the coal
disturbed are with the permit boundary on the map. This map should be updated with
correct boundary delineations as soon as possible and submitted to DRMS for review as
per Rule 2.10.

ROADS- Rule 4.03
Construction 4.03.1(3) I 4.03.2(3); Drainage 4.03.1(4) I 4.03.2(4); Surfacing and Maintenance
