Newmont Mining Corporation <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,June 11,2016 <br /> 2.3 Geomembrane Installation <br /> Geomembrane installation continued within the Phase 1 SGVLF in the areas shown on Figure 3. <br /> Approximately 102,000 square feet (P-1732 to P-1795) of 80 mil LLDPE (double sided texture) <br /> was deployed. Subgrade was inspected and approved by NewFields and ECA representatives <br /> prior to geomembrane deployment. Approximately 5,183 linear feet of seam was fusion welded <br /> using two fusion welding machine/operator combinations. Eleven destructive testing samples <br /> were marked during fusion seaming. Approximately 871 linear feet of seam was extrusion <br /> welded using two extrusion welding machine/operator combinations. Four destructive testing <br /> samples were marked during extrusion seaming. All destructive tests performed met project <br /> specifications. Non-destructive testing was performed for fusion welded seams. Repairs were <br /> performed using the extrusion welding method for all marked defects. Vacuum testing was <br /> performed for all extrusion welded seams and repairs. <br /> 2.4 Anchor Trench <br /> Approximately 800 feet of permanent anchor trench was excavated along the Phase 1 <br /> perimeter in accordance with design drawings and project technical specifications. <br /> Anchor trench backfill was performed using an excavator, a vibratory plate compactor and <br /> laborers. A NewFields representative monitored the backfill procedures to ensure that the <br /> anchor trench was constructed in accordance with the design drawings and project technical <br /> specifications. <br /> 2.5 Geomembrane Acceptance <br /> Panels P-1716 through P-1731 were accepted for Drain Cover Fill placement. The attached <br /> Figure 3 shows the acceptance areas. <br /> 3.0 STORM WATER MANAGEMENT <br /> Best Management Practices continued to be performed. <br /> 4.0 NEWFIELDS LABORATORY ACTIVITIES <br /> NewFields personnel performed sieve analysis, atterberg limits, compaction and permeability <br /> testing.The following samples were collected for laboratory testing: <br /> ➢ Drain Cover Fill: NFDCF-4-R (Phase 1 VLF) <br /> Soil Liner Fill: NFSLF-23-C and NFSLF-24-C (Dump 1 Processing Plant) <br /> Page 4 <br />
