June 7, 2016 C-1981-012/New Elk Mine RDZ <br /> <br /> <br />Number of Partial Inspection this Fiscal Year: 10 <br />Number of Complete Inspections this Fiscal Year: 4 <br /> <br /> Page 4 of 19 <br /> <br />On the steep slope to the east of Pond 08 are large bare areas and rills. NECC should consider options for <br />addressing this source of sediment. <br /> <br />No problems were seen on the bank between the SAE that includes Topsoil Stockpile #2 and the river. The berm <br />that control runoff from the SAE is intact. However, the soil that was piled on the berm needs to be removed. As it <br />is currently placed, it could slump onto the river bank. <br /> <br />The following culverts were found to be large enough (per the Table 21 specifications) and mostly free of <br />sediment: C-B (on RDA road), C4, C12A, C21, C39, and C49. <br /> <br />Some issues were identified with culverts: <br />A culvert was found east of the substation (that is south of the office building), but this is not on Map 13. <br />Culvert C-A needs to be cleaned (or did at time of inspection). <br />Culvert C-B is not listed in Table 21. <br />Culvert C11A is on Map 13 but was not found in the field. <br />Culvert C14 is not listed in Table 21. <br />Culvert C23 is mostly free of sediment but is smaller than the specification. <br />Two culverts labeled C52 are shown on Map 14. The one to the west (south of Containment #2) does not appear to <br />exist. The map should be updated. <br />Culvert C64 was almost completely plugged; after the inspection, NECC sent an email to Rob Zuber indicating <br />that the maintenance work had been performed. <br /> <br />The following ditches were inspected: D2, D3, D4, D10, D10A, D11, D12, D14, D15, D19, D28, D30, D32, <br />D32A, D39A, D39AA, D55, D56, D57, D61, D62, and D63. <br /> <br />The following issues were identified with ditches: <br />Ditch D10A needs to be cleaned to allow for positive drainage and prevent puddles (as is in the ditch now). <br />Ditch D11 is poorly marked on Map 13. It appears to be the channel that drains the SAE that is east of the <br />DWDA#2, but that is not obvious per the map. <br />The channel that drains the SAE that is east of the DWDA#2 (apparently ditch D11) is eroding at the lower end. <br />Ditch D28 is eroding below culvert C49. Also, this ditch is discontinuous, as there is a structure that routes the <br />flow toward the river rather than to the east as shown on the map. Per Ron Thompson, the noted structure is <br />related to the old railroad bed. Finally, this ditch drains to Containment #2, however the map indicates otherwise. <br />Ditch D39A is silted in. It should be cleared or the map and Table 20 should be altered to reflect the ground <br />condition. There are no erosion problems at this ditch near the parking lot and substation. <br />Ditch D39AA was not found in the field. <br />The upper portion of ditch D56 requires maintenance. <br />Ditch D61 appears to be too narrow per the specification in the PAP (Table 20). <br />Ditch D63 requires maintenance. <br />There is no label on Map 14 for the ditch that is east of the two SAEs near the west end of the mine site (formerly <br />the tank pad and substation). <br />
