PERMIT #: M-2007-030
INSPECTOR’S INITIALS: ACY
INSPECTION DATE: May 27, 2016

It was mentioned that a sediment pond will be created in the northern portion of the permit boundary.
Likewise the existing dirt road to the water treatment plant has been upgraded and will soon be paved. Please
make sure that the mining and reclamation maps are actuate. The Division cannot release a site if the as -built
site conditions do not match the approved reclamation maps.

The post mining land use for this site is industrial commercial. All final reclamation efforts shall support this
land use until released from DRMS jurisdiction. It was mentioned that this site may potentially be used as a
park. Recreational land use needs to occur after release of DRMS permitting. Maps should indicate which
roads and buildings are to remain upon reclamation. The approved reclamation plan calls for 3:1 slopes that
are disked, broadcast seeded and straw mulched. DRMS will specifically evaluate eros ion, slope stability and
potential for off-site damage. The vegetation required is related to stability not a specific density. If areas will
not be seeded please make sure this is distinguished on the map. Stability requirements still apply to areas not
seeded.

The operator had posed questions regarding permit status. There are four categories of mining; active,
intermittent operator, temporary cessation and final reclamation. Per Rule 1.13 active status requires mining
more than 180 days a year. Intermittent status requires that a site be active year around however does not
operate more than 180 days a year. If the mine ceases production for more than 180 days it shall go into
temporary cessation. And if a site has finished mining it shall being final reclamation. Currently the site is
active according to the Divisions records. To change your permit status please complete the appropriate
revision form. Commencement of final reclamation only requires written notice.

Any minor changes to the permit shall be done in the form of a technical revision with the required $216 fee.
Please note that several minor changes can be done under one technical revision.

No problems or violations were noted during this inspection.

Responses to this inspection report should be directed to: Amy Yeldell at the Division of Reclamation, Mining
and Safety, 1313 Sherman St., Room 215, Denver, CO 80203.
