Climax Mine Closure Documentation <br /> CAVR Asbestos Disposal Shaft May 11, 2016 <br /> north, to shed water off of the cap. A brass monument was embedded in the concrete identifying the shaft <br /> as an asbestos disposal site (Photo 6; Attachment 1). The corners of the building foundation were also <br /> surveyed and have the following latitude/longitude coordinates and elevations: <br /> Latitude Longitude Elevation (feet) <br /> 39-21-46.1531 106-10-23.0048 11,583.59 <br /> 39-21-46.0189 106-10-22.8566 11,583.53 <br /> 39-21-45.823 106-10-23.1426 11,583.58 <br /> 39-21-45.9600 106-10-23.2905 11,583.67 <br /> Construction work was observed by Climax and Tetra Tech staff and a final inspection of the work was <br /> performed by Tetra Tech on August 12, 2015, prior to concrete placement. All formwork and rebar <br /> placement was observed to be in place as designed and specified. During concrete placement,field quality <br /> control testing was performed including field testing of slump and entrained air. In addition, two sets of <br /> four test cylinders were cast on-site to verify the compressive strength of the concrete. One cylinder, from <br /> each set, was tested at seven days after placement, and three at twenty-eight days. The results of the field <br /> quality control tests and compressive strength results are presented on the test reports provided in <br /> Attachment 6. These results indicate the achievement of 28-day compressive strengths in excess of 7,400 <br /> psi, meeting the specified design strength of 6,000 psi. <br /> Based on observations made during construction and documentation provided, the construction of the <br /> CAVR concrete shaft cap has been completed in satisfaction of the requirements of the approved Closure <br /> Plan. <br /> 1 "PROFESSIONAL CERTIFICATION <br /> I herein certify that, to the best of my knowledge and based on observations, documentation provided, and <br /> people interviewed, the construction of the concrete shaft cap for the CAVR Shaft at the Climax Mine has <br /> been performed in accordance with the approved specifications and engineering plans in general <br /> accordance with the approved November 16, 2012 closure plan.
