PERMIT #: M-1983-194 <br />INSPECTOR’S INITIALS: THM <br />INSPECTION DATE: May 4, 2016 <br /> <br /> <br />Page 2 of 4 <br /> <br />OBSERVATIONS <br /> <br />This partial inspection was conducted by the Colorado Division of Reclamation, Mining and Safety in response to a <br />Technical Revision request provided by Natural Soda, Inc. on April 26, 2016. The proposed 15HH – SSMW (Subsurface <br />Subsidence Monitoring Well) was discussed and observed during the inspection. Kirk Daehling of Natural Soda and Jerry <br />Daub of Daub and Associates were both present during the inspection. <br /> <br />The 15HH – SSMW is located on a reclaimed but previously disturbed 4-3H well pad which is adjacent to the 8H pad. <br />The proposed access road will access the 4-3H pad from the 8H Pad. <br /> <br />The recently constructed pad location for well pairs 15H, 16H and 17H was observed during the inspection. The pad was <br />constructed in a terrace fashion. Gravel was placed on the pads. The access for the pad also stems off of the 8H Pad. The <br />access road crosses a few significant drainages. The drainage crossings appeared to be well constructed and had what <br />appeared to be adequate rip rap placed to minimize erosional impacts to the structures and drainage. <br /> <br />It was observed that the overall condition of the plant and well pad areas were free of any unecessary debris. Mr. <br />Daehling stated that one of the goals for Natural Soda is to clean up all unused debris. The DRMS sees this as a good <br />house keeping practice and an overall good mining ethic. <br /> <br />No off-site damage was observed during the inspection. <br /> <br />No problems or possible violations were observed during the inspection. <br /> <br />Photos of the conditions observed during the inspection are located at the end of this inspection report. <br /> <br />If you need more information regarding this inspection report please contact Travis Marshall in the DRMS Grand Junction <br />Field Office at (970) 241-2042. <br /> <br /> <br /> <br />
