Newmont Mining Corporation • <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CCW <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,May 14,2016 <br /> Approximately 5,900 square feet of geogrid was placed for the third layer at the fourth step of <br /> the lower Anaconda Mine Complex. <br /> 2.2 Geomembrane <br /> ECA performed repairs on previously installed, non-certified geomembrane within Phase 1 <br /> between stations H17+00 and H21+00 and H24+00 to H28+00. Ames worked to clean the <br /> geomembrane of all deleterious material prior to ECA performing repairs. Nondestructive <br /> testing was completed on all repairs. <br /> The geomembrane that is being repaired was placed at the end of 2015, and was left exposed <br /> to potential damage over the winter. All damages are on geomembrane that was previously <br /> approved for DCF placement. No new defects were marked; however, a NewFields <br /> representative was present for all repair activities. When repair activities are complete, <br /> NewFields personnel will inspect all of the exposed geomembrane prior to reapproving it for <br /> DCF placement. <br /> 3.0 STORM WATER MANAGEMENT <br /> Best Management Practices continued to be performed. <br /> 4.0 NEWFIELDS LABORATORY ACTIVITIES <br /> NewFields personnel performed sieve analysis, atterberg limits, compaction and permeability <br /> testing. Two samples of Soil Liner Fill (NFSLF-15-C and NFSLF-16-C) were collected from the <br /> Dump 1 Processing Plant, and a record sample was collected for Structural Fill (NFSF-3-R). <br /> 5.0 MEETINGS AND COMMUNICATIONS <br /> NewFields personnel attended the weekly Contractor Planning Meeting (May 10 2016), weekly <br /> Contractor Safety Meeting (May 12, 2016), Ames daily safety meetings and NewFields daily <br /> safety meetings. <br /> 6.0 MISCELLANEOUS, ISSUES AND CONCERNS <br /> NewFields did not have any survey data for the fourth step of the Lower Anaconda Mine <br /> Complex that was constructed prior to NewFields arriving on site. The survey data was <br /> requested from Foresight West and Ames. The survey data was only provided for two <br /> layers of geogrid; however, the remediation plan requires that each step be three <br /> layers. Ames excavated a small portion of the fourth step area to determine if the third <br /> layer of geogrid was placed but never surveyed. It was concluded that only two layers of <br /> geogrid had been installed during 2015. Ames worked to remove a portion of the fifth <br /> Page 4 <br />
