Newmont Mining Corporation a■ <br /> Cripple Creek&Victor Gold Mine . <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,May 14,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> • NewFields personnel observed confirmatory drilling activities and noted any voids that <br /> were encountered. Confirmatory drilling was performed at the following underground <br /> workings: <br /> o Anaconda Mine Complex(further remediation required) <br /> • Investigation of Underground Working#6678 was performed. The area was blasted and <br /> the excavator is preparing the working for a concrete plug. Further remediation is <br /> required. <br /> • Blasting was performed at Underground Working#6477. Three shafts were collapsed by <br /> loading confirmatory boreholes. Production blast holes were also shot to achieve the <br /> necessary depth for geogrid placement below proposed bottom of Soil Liner Fill. Further <br /> remediation is required. <br /> • Excavation for the exposure of the fourth geogrid step at the Lower Anaconda Mine <br /> Complex was completed. Approximately 1/3 of step five was salvaged and will be tied <br /> into as remediation continues. The top of the SSF above the second geogrid layer of the <br /> fourth step was re-compacted. The third layer of geogrid was placed and surveyed. <br /> Select Structural Fill was placed over the third layer and compacted using a smooth <br /> drum roller. Further remediation is required at the Lower Anaconda Mine Complex. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase I and Phase II footprint. <br /> NewFields personnel inspected fill material and observed placement and compaction <br /> procedures to ensure that they were in accordance with the project technical specifications. <br /> Ambient temperatures remained above 32 degrees Fahrenheit throughout the shift. The <br /> following structural fill construction activities occurred during the shift: <br /> • Slope rough grading(cut to fill) in areas bounded by bench stations: <br /> o K23+00 to K38+00 and M22+00 to M30+00 <br /> o K15+00 to K18+00 and 119+00 to 121+00 <br /> o M30+00 to M37+00 and 535+00 to 543+00 <br /> • Slope rough grading(cut to stockpile) in areas bounded by bench stations: <br /> o K19+00 to K23+00 and M15+00 to M19+00 <br /> o 127+00 to 134+00 and K24+00 to K29+00 <br /> Page 2 <br />
