Box 806 <br /> Salt Lake City, UT 84110 <br /> RE: Last Call for Delinquent Annual Report and Map, Camp Bird Mine, Permit No. M-1982-090 <br /> Dear Mr. Butters: <br /> Pursuant to C.R.S. 34-32-116(3) and (4), and the conditions of Permit No. M-1982-090, you are <br /> required to submit$633 in annual fees and an annual report and map by the anniversary date of <br /> the Permit, by December 71"of each year. As of the elate of this correspondence,the Operator is <br /> current regarding the payment of the annual fees. However, the Division has not received the <br /> annual reports and maps for two consecutive years, due December 7, 2014 and December 7, 2015. <br /> Given the unique circumstances of this specific site, the Division has granted a limited grace period <br /> for receipt of the delinquent annual reports and maps, now due June 20, 2016.
