COLORADO <br />Division of Reclamation, <br />Mining and Safety <br />I Dtoai:men, of Ndturai RcxOuree; <br />1313 Sherman Street, Room 215 <br />Denver, CO 80203 <br />May 18, 2016 <br />Interoffice Memo <br />Trapper Mine (Permit No. C-1981-010) <br />Interoffice Memo - Horse Gulch Fill Quarterly Fill Certification — 41 Qrt 2015 —1St Qrt 2016 <br />Tabetha, <br />After my review of the Trapper Mine's Horse Gulch Fill Quarterly Fill Certification — 4th Qrt 2015 <br />— 1St Qrt 2016, I have found the above referenced reports to be in general complianceof Rule <br />4.09.1(11): <br />4.09.1(11)(a) Inspected at least quarterly by a registered engineer or other qualified professional <br />specialist in construction of earth and rockfill embankments under the direction of <br />a qualified engineer throughout the construction. <br />The certified reports cover the 4th quarter monitoring period from October to <br />December of 2015 as well as the 1St quarter monitoring period from January to <br />March 2016. <br />4.09.1(11)(b) Provide to the Division a certified report within 2 weeks after each inspection that <br />the fill has been constructed as specified in the design approved by the Division. <br />The report shall include a description of any appearances of instability, structural <br />weakness, and other hazardous conditions. <br />The inspections were conducted on December 10, 2015 (4th quarter 2015) and <br />March 31, 2016 (1St quarter 2016). The Division received the reports on <br />December 21, 2015 and April 8, 2016 respectively. No instability, structural <br />weakness, or other hazardous conditions were noted during these inspections of <br />the fill. The area where cracker were detected on the top south portion of the fill <br />during the first quarter of 2013 was inspected during both quarters.
