Box 531 Mancos, CO 81328 Re: Response to Request for Annual Fee Status, Notice of Intent, Notice No. P-2012-007 and P-2009-018 Mr. Liukko, On May 4, 2016 the Division of Reclamation, Mining and Safety (Division) received a correspondence from you, for the above listed Notices of Intent (NOI), regarding the annual fee and report request. Our records indicate that despite having been fully reclaimed, no formal release request was filed for either Notice. In order to release a NOI and its associated financial warranty, the Operator must formally submit a release request in compliance with Hard Rock and Metals Mining Rule 4.16. Please note that release of the site and its associated financial warranty will not be considered until all annual fees and reports are up to date. The outstanding and pending annual fees are as follows; 1. P-2012-007 a. 2015 Annual Fee, Past Due = $86.00 b. 2016 Annual Fee, Due 5/17/2016 = $86.00 2. P-2009-018 a. 2016 Annual Fee, Due 6/24/2016 = $86.00 If the Annual Fees and Reports for NOI No. P-2012-007 are not received within 30 days enforcement action will be pursued, and the item will be scheduled before the Colorado Mined Land Reclamation Board for revocation and financial warranty forfeiture. All fees and reports for compliance must be received by the Division’s Denver Office no later than Monday June, 13 2016. Similarly, if the Annual Fee and Report for NOI No. P-2009-018 are not received by the respective due date, enforcement action will be pursued as well.
