PERMIT#: M-1977-002-HR <br /> INSPECTOR'S INITIALS: MAC <br /> INSPECTION DATE:January 20,2016 <br /> OBSERVATIONS <br /> The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br /> (Division). Holly Lien and Rob Kisner of Colorado Lien Company were also present for the inspection.The <br /> Munroe Gypsum Quarry is located approximately 6 miles north of Livermore, Colorado. The site is permitted <br /> for 240 acres with an affected area of 70.7 acres. The Operator has committed to a maximum disturbed area <br /> of 9.9 acres.The post-mining land use is rangeland. <br /> Financial Warranty: <br /> The Division currently holds a financial warranty in the amount of$34,800.00.The financial warranty has not <br /> been adjusted in over ten years. Therefore, the Division will perform a full bond calculation to ensure the <br /> financial warranty remains adequate to complete reclamation. Notice of a financial warranty increase will be <br /> sent under separate cover. <br /> Gen. Compliance With Mine Plan: <br /> The site was active at the time of the inspection.The Operator periodically drills and blasts within the quarry <br /> and hauls the gypsum to an offsite facility.The gypsum deposit is approximately 35 feet thick and the <br /> overburden varies in thickness.The highwall is on average 40 feet in height and 1,100 feet in length. Mining is <br /> currently progressing to the north and west. <br /> Reclamation Success: <br /> During the previous year, the Operator performed reclamation on approximately 2 acres located west of the <br /> active quarry area. The reclaimed area was backfilled to a final grade of approximately 3H:1V. The area was <br /> also seeded and mulched; the Division did not observe germination of any of the planted grass species. <br /> Sediment Control: <br /> The Division observed sediment fences along the south side of the access road which were no longer anchored <br /> in the ground. The Division did not observe sediment leaving the permit area. However,the failure to <br /> maintain stormwater controls has been cited as a problem and will require corrective action by the <br /> Operator. Please see the first page of this report for additional information.* <br /> Signs and Markers: <br /> The permit sign was posted at the site entrance as required by Rule 3.1.12. However, several of the southern <br /> boundary markers were missing.The missing boundary markers have been cited as a problem and will <br /> require corrective action by the Operator. Please see the first page of this report for additional <br /> information.* <br /> This concludes the inspection. <br /> *The Operator corrected the problems cited in this inspection report before the inspection report was mailed. <br /> All problems which were cited have been resolved. <br /> Page 2 of 4 <br />
