PERMIT#:M-2002-004 <br /> INSPECTOR'S INITIALS: TOD <br /> INSPECTION DATE:November 13,2015 <br /> to the Division in the annual report. The Division believes more measures need to be taken to monitor <br /> groundwater quality, The Division will continue to work with the Operator on a revised groundwater monitoring <br /> plan. <br /> Gen. Compliance With Mine Plan: <br /> The operation appeared to be following the approved mine plan. All mining activity appeared to be within the <br /> approved boundary. <br /> Note: The Division observed an area located around the cement kiln where new a structure(s)would possibly be <br /> built. The new a structure(s) will support the new dust collection system. Pursuant to Rule 6.4.3 the Operator <br /> shall maintain a record on the Mine Plan Map of all structures located within the affected lands. If the Operator <br /> plans on adding new structures to the facility the Operator will need to revise the approved reclamation permit <br /> and submit a new Mine Plan Map and/or a Reclamation Plan Map. The revised Mine Plan map and/or <br /> Reclamation Plan Map shall be added to the approved permit through a Technical Revision. <br /> Roads: <br /> All haul roads were stable and well maintained. The haul roads were being watered during the inspection. No <br /> excessive dust was observed from mining activities. <br /> Reclamation Success: <br /> The Operator has successfully graded the western portion of the current affected lands to 4H:IV or flatter. The <br /> final graded portions of the site appear stable, no large erosion features were observed during the inspection. <br /> Approximately 56 acres along the western portion of the site have had various attempts to establish vegetative <br /> cover. Desired vegetative cover is relatively sparse throughout much of the seeded areas. <br /> Reve etg ation: <br /> There was plant growth on the stockpiles and throughout the rest of the affected area. The growth in the affected <br /> areas appeared to be volunteer vegetation,comprised primarily of annual weeds,mainly Russian thistle and kosha. <br /> Desired vegetative cover was relatively sparse in portions of the seeded areas. The Operator has attempted to <br /> reestablish vegetative cover on approximately 56 acres, yielding mixed results. In portions of the 56 acres <br /> vegetative cover has begun to establish well (photo 6). In other portions of the revegetated area, the areas will <br /> either need to be re-seeded or partially re-seeded. The vegetative cover included a variety of life forms and <br /> species. Herbaceous species observed during the inspection include: a variety of wheatgrasses, sand dropseed, <br /> sideoats grama, and various other forbs were scattered throughout the revegetated areas. <br /> Based on the sparse desired vegetative cover observed during the inspection, the Division advised the Operator <br /> to consider amending the seed mix to include species that are more tolerant to alkali conditions. During the <br /> inspection the Division and Operator had a discussion about various revegetation methods. The Division also <br /> advised the Operator to consider using some form of a cover crop; like Triticale (sterile)(mentioned in approved <br /> permit), Sorghum, Millet, Oats, and/or Barley. The Division also advised the Operator to mow annual weeds <br /> prior to seed production, to prevent the spread of annual weeds. The Operator is advised to continue to monitor <br /> noxious weeds and treat noxious weeds if they appear within the permit boundary. <br /> Note: If the Operator modifies the revegetation plan, the Operator will need to submit a Technical Revision to <br /> update the information require by Rule 6.4.5. <br /> Erosion/Sedimentation: <br /> There was no evidence of excessive erosion (rills, gullies, or sediment fans). <br /> Page 3 of 7 <br />
