PERMIT#: M-1997-098 <br /> INSPECTOR'S INITIALS: THM <br /> INSPECTION DATE: April 12, 2016 <br /> OBSERVATIONS <br /> This inspection was conducted as a part of the normal program established by the Colorado Division <br /> of Reclamation, Mining and Safety (DRMS). The Flag Creek Quarry is a 112c permitted site located <br /> approximately 10 miles south of Meeker, Colorado off of Rio Blanco County Road 13. Shawn Bolton <br /> and Joe Barton and were present during the inspection. <br /> Mr. Bolton is considering taking over the operation and wanted to observe the site with DRMS staff <br /> to understand what compliance requirements and if any issues were associated with the operation. <br /> The mine identification sign was in place and in compliance with rule 3.1.12. The sign is posted on <br /> the fence located at the main entrance on the west side of the Flag Creek Quarry. White PVC <br /> boundary markers were observed around the entire perimeter of the 5.028 acre staging area located <br /> adjacent to Rio Blanco County Road 13. <br /> The Quarry was inactive at the time of the inspection. The upper quarry area of the permit area was <br /> inaccessible due to snow drifts restricting travel on the haul road. The quarry was observed from <br /> the bottom from Rio Blanco County Road 13. The face of the slope had various road cuts that worked <br /> their way up the slope to the top of the quarry. According to the reclamation permit access to the <br /> quarry is permitted from the bottom of the slope via the existing haul road. I have included the <br /> approved mining and reclamation plan that specifies all compliance requirements for the operation. <br /> The process area had various pallets of flagstone ready for sale. A Cat D9 dozer was present on the <br /> site along with various pieces of rock cutting equipment. No activities were observed during the <br /> inspection and Mr. Bolton stated that the last time the site was operating was in 2015. <br /> Two fuel storage tanks were observed without secondary containment within the process area. The <br /> lack of secondary containment is considered a problem and must be corrected. Please supply photo <br /> documentation that all fuel containers are stored properly - including applicable secondary <br /> containment structures by the corrective action date of May 31, 2016. Note that secondary <br /> containment structures shall consist of an impermeable containment which could contain all contents <br /> of the tanks and various containers (when full) plus 10% of the total capacity. The operator may also <br /> provide photo documentation that all containers have been removed from the site on or before the <br /> corrective action date. <br /> In an effort to ensure the financial warranty remains adequate to reclaim this site per the <br /> requirements of the approved reclamation plan, DRMS will update the reclamation cost estimate. <br /> The estimate will be forwarded to the operator under separate cover. <br /> Photos that show the conditions observed during the inspection are included at the end of this report. <br /> Responses to this inspection report should be directed to Travis Marshall at the Division of <br /> Reclamation, Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand <br /> Junction, Colorado 81501, phone no. 970-623-5596. <br /> Page 2 of 6 <br />
