y -
t---APR 18 2016
DIVISION OF RECLAMATION
April 13, 2016 MININGANDSAFETY
Mr. Tyler O'Donnell
Division of Reclamation Mining and Safety
1313 Sherman Street, Room 215
Denver, CO 80203 AGGREGATE
L- INDUSTRIES
Re: Permit Number M-2004-031 —Hazeltine Mine
Amendment AM-0I Pre-Op Inspection - Corrective Action Response
Dear Tyler:
Aggregate Industries (AI) submits this letter and enclosures in response to the corrective action
item regarding clearing blockages of the temporary drains as set forth in the Division's report of
its March 3, 2016 inspection at Hazeltine. As of March 15, the blockage had cleared
significantly and the inlets of both pipes were visible. Water appeared to be discharging through
both outlets at that time. On April 5,the pipes were still functioning and water level at the pipe
inlets was significantly reduced. Pictures of these two monitoring events are enclosed as
evidence of the cleared pipes.
A screen in front of the pipes to stop blockages from entering has not been installed; however,
the pipes are functioning and draining the property, as evidenced in the April 5 photo of the staff
gauge that is located east of the drain pipes. Appropriate screening measures will be
incorporated into the permanent drain plan.
Sometime between March 1 and March 15, Fulton Ditch stopped draining water from their ditch
through the pipe that discharges at the south boundary between AI's property and the Orr
property. This has significantly reduced the amount of water in the Bull Seep and around the Orr
property.
Verytruly yours,
Connie N.
