TETRA TECH TECHNICAL <br /> MEMORANDUM <br /> To: Brian McGill <br /> Company: Climax Molybdenum Company—Climax Mine <br /> CC: <br /> From: Daryl Longwell, P.E. <br /> Date: January 15, 2016 <br /> Subject: Climax Mine, CAVR Asbestos Disposal Shaft—Closure Documentation <br /> Project No. 114-910345 <br /> The purpose of this report is to summarize the work performed to complete the closure of the Crusher Adit <br /> Vent Raise (CAVR) Shaft at the Climax Mine, and to provide certification of completion. The CAVR Shaft <br /> is a former ventilation shaft that was converted for use as an asbestos disposal shaft, pursuant to approval <br /> from the Colorado Department of Public Health &Environment(CDPHE), Hazardous Materials and Waste <br /> Management Division. Closure activities have been performed in general accordance with the approved <br /> November 16, 2012 closure plan titled "Climax Mine Crusher Adit Vent Raise Asbestos Disposal Shaft <br /> Closure Plan." <br /> In summary, the closure activities included the execution of the following steps: <br /> 1. Decontamination and Deconstruction of the Enclosure Building <br /> • Prior to the physical closure of the shaft,the enclosure building will be decontaminated <br /> by a licensed abatement contractor in accordance with the asbestos decontamination <br /> work plan for the CAVR shaft dated November 14, 2012. <br /> 2. Construction and placement of formwork and steel reinforcement(rebar); and <br /> 3. Construction of reinforced concrete cap to seal the shaft and have the ability to support loading of up <br /> to 100 feet of overburden material; and <br /> 4. Placement of a brass monument, embedded in the concrete, identifying the site as an asbestos <br /> disposal site. <br /> The following sections describe the work performed, referencing various documentation included as <br /> attachments to this report. Also included, as Attachment 1, is a Photo Log, providing photographic <br /> documentation of work progress. <br /> 1.0. DECONTAMINATION AND DECONSTRUCTION <br /> In general accordance with the Asbestos Decontamination Work Plan, attached to the above-referenced <br /> Closure Plan, the interior of the building was decontaminated, and the HEPA fan exhaust system and bag <br /> disposal conveyance system (disposal chute) were dismantled and removed to prepare the building for <br /> demolition and shaft cap construction.All asbestos abatement work was performed by NorthStar Demolition <br /> and Remediation, Inc. (NorthStar),formerly known as LVI Environmental Services, Inc., a certified General <br /> Abatement Contractor in the State of Colorado. Abatement work began on August 11, 2014 and final <br /> clearance air sampling was performed on August 19, 2014.
