PERMIT #: M-1981-080 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: April 28, 2015 <br /> <br /> <br />Page 2 of 6 <br /> <br />OBSERVATIONS <br /> <br />This inspection was conducted as part of the regular monitoring program. The Operator (Tezak Stone & Aggr <br />Supply, Inc.) was represented by Messrs. Mike and Nathan Tezak who were present for the inspection. <br /> <br />The Tezak No 2 Pit site access road entrance is located off US 50 approximately 30 miles west of Cañon City. <br />This is a 110c gravel mine. The mine was not active at the time of the inspection. <br /> <br />Inspection: <br />Markers & Boundary – A permit sign was displayed at the site entrance (see Photo 1). The Operator has marked <br />the limits of mine excavation with T-posts (see Photo 2) and large boulders (see Photo 3). The north and east <br />permit boundaries were marked with fencing (see Photos 4). <br /> <br />Mining Plan – Working highwalls were observed to be near 1.5H:1V (see Photo 5), but finished area highwalls <br />were flattened to at least 3H:1V (see Photo 3) and some older areas vegetated (see Photo 6). Several stockpiles <br />of recently crushed material were observed on the pit floor. Mr. Tezak indicated he just had a contractor come <br />in to crush a 3 to 5-year supply of material. <br /> <br />Reclamation – Some highwall reclamation had been completed as previously mentioned. Topsoil stockpiles <br />were placed west of the working highwall (see Photo 5) to be used for concurrent reclamation. <br /> <br />Environmental – No problems with contaminant spills, erosion/sedimentation or noxious weeds were observed. <br /> <br />Records: <br /> Revisions: Amendment (AM-01) to add 2.7 acres in 1995, permit transfer (SO-01) in 1995, and technical <br />revision (in 2000) to allow a batch plant. <br /> Commitments/Stipulations: none. <br /> Other Records <br />a) The total permitted area is 9.9 acres, the 2014 annual report indicated 9.9 acres of disturbance which <br />is consistent with Google Earth disturbed area estimates. <br />b) The previous inspection was performed on 6/7/2010. No problems were cited. <br />c) The anniversary date is May 27. Annual reports and fees are current through 2015. <br />d) The post-mine land use is rangeland. <br /> <br />Bond: <br />The Division holds a $17,500 bond which is deemed adequate for the current disturbance . A cost summary <br />sheet is attached. Detailed bond estimates can be made available. <br /> <br />Summary and Recommendations: <br />1. No problems or concerns were observed during the inspection. <br />2.
