Cripple Creek & Victor Gold Mining Company <br />Squaw Gulch VLF Phase 1 <br />(9,550-foot Elevation Bench to Geomembrane Panel P1635) <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 2016 <br />Project No.: 74201125N0 Page 24 <br />Prior to the 80-mil LLDPE DSMS geomembrane arrival to site, Amec Foster Wheeler <br />performed 49 conformance tests on the geomembrane, at a frequency of one test per <br />145,294 square feet. The conformance testing includes all of the geomembrane on-site, <br />not just what was used within this Project boundary. Conformance testing for the 80-mil <br />DSMS LLDPE geomembrane used to construct the liner system met project technical <br />specifications. Conformance test results for the 80-mil DSMS LLDPE are presented in <br />Appendix K.1. <br />Prior to the 40-mil LLDPE smooth geomembrane arrival to site, Amec Foster Wheeler <br />performed 1 conformance test on the geomembrane, at a frequency of one test per 145, <br />square feet. The conformance testing includes all of the geomembrane on-site, not just <br />what was used within this Project boundary. Conformance testing for the 40-mil smooth <br />LLDPE geomembrane used to construct the liner system met project technical <br />specifications. Conformance test results for the 40-mil smooth LLDPE are presented in <br />Appendix K.2. <br />6.2 Deployment Monitoring <br />Geomembrane panel deployment monitoring performed by Amec Foster Wheeler <br />included: <br />• Observation of panel surface condition and panel overlap <br />• Assignment of panel numbers <br />• Verification of factory roll number and receipt of complete certification prior to <br />allowing deployment <br />• Measurement of sheet thickness <br />• Documentation of deployed panel lengths <br />• Development of panel layout record drawing <br />Prior to the deployment of geomembrane, Amec Foster Wheeler and ECA verified that <br />the SLF surface was free of soft spots, protrusions, angular particles, abrupt grade <br />changes, and loose soil. Amec Foster Wheeler and ECA also verified the surface had <br />been proof-rolled and was free of desiccation cracks. Where the surface of the SLF did <br />not meet project technical specifications, Ames repaired the area and re-compacted the <br />surface. Amec Foster Wheeler prepared SLF surface acceptance forms. After <br />inspections of the finished surface, an Ames, Amec Foster Wheeler and CC&V <br />representative signed the acceptance forms. The SLF surface acceptance certificates <br />are presented in Appendix G. <br />Amec Foster Wheeler assigned each geomembrane panel a number. Geomembrane <br />panels were designated with a “P” followed by an individual panel number. Some panels
