Cripple Creek & Victor Gold Mining Company
Squaw Gulch VLF Phase 1
(9,550-foot Elevation Bench to Geomembrane Panel P1635)
Final Report
Quality Assurance Monitoring & Test Results
January 2016
Project No.: 74201125N0 Page 22
project requirements. Geomembrane was placed only when complete QC
documentation was provided and determined the test results met or exceeded project
technical specifications. The 40-mil LLDPE smooth geomembrane QC roll certificates
are presented in Appendix I.5 and the 80-mil LLDPE DSMS geomembrane QC roll
certificates are presented in Appendix I.4. Please note the 80-mil LLDPE DSMS titles at
the top of the roll certificates in Appendix I.4 are called out three different ways
(“Microspike LL 80”, “MicroSpike™ LLDPE”, and “80 LL Microspike”) all refer to 80-mil
LLDPE DSMS.
Welding Rod QC Certificates
Welding rod certification consisted of resin QC documents for the resin used to
manufacture the extrusion welding rod material for the Project. The welding material
used for the liner was produced from four batches of resin. The resin lots used to
produce the welding rod resin were not from the same resin lots used to produce the
80-mil LLDPE geomembrane, but were produced from the same resin. Amec Foster
Wheeler’s review of the QC certification indicates the resin used to manufacture the
welding material met project technical specifications. The welding rod QC certificates
are presented in Appendix I.8.
5.3 Geotextile QC Documents
Geotextile, manufactured by TenCate, was used during construction of the underdrain
systems as well as the LDS. Amec Foster Wheeler’s review of the QC certificates
indicates the geotextile met project technical specifications. Geotextile QC certificates
are presented in Appendix I.9.
