Cripple Creek & Victor Gold Mining Company <br />Squaw Gulch VLF Phase 1 <br />(9,550-foot Elevation Bench to Geomembrane Panel P1635) <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 2016 <br />Project No.: 74201125N0 Page 14 <br />4.0 EARTHWORK CQA TESTING AND MONITORING <br />The following sections summarize results of the earthwork CQA testing and monitoring <br />performed by Amec Foster Wheeler. <br />Amec Foster Wheeler performed CQA testing of in-place materials used for construction <br />of the Project. Table 4 presents specified and observed CQA testing frequencies. <br />Figure 2 shows the construction and geomembrane installation limits and coordinate <br />system used to locate tests. <br />In accordance with the AMEC Earthworks CQA Plan (Section 1400.1 of the project <br />technical specifications in Appendix E), when an Amec Foster Wheeler monitor <br />observed deficiencies, the monitor determined the nature and extent of the problem and <br />notified Ames and/or CC&V. Amec Foster Wheeler then performed additional testing to <br />define the extent of the deficient area. Ames corrected deficiencies to meet the <br />requirements of the project technical specifications and Amec Foster Wheeler re-tested <br />the corrected area(s) prior to any additional related work. The following sections include <br />discussions of results encountered. <br />Discussions in Section 4.0 and subsequent subsections represent testing within the <br />Project boundary unless otherwise noted. <br />4.1 Results of Laboratory Soils Testing <br />Amec Foster Wheeler performed laboratory testing of soils during all earthworks aspects <br />of the construction at Amec Foster Wheeler’s on-site soils laboratory. To verify that the <br />earthwork materials used during construction met project technical specifications, Amec <br />Foster Wheeler performed the following tests in accordance with the American Society <br />for Testing and Materials (ASTM): <br />• Grain size distribution (ASTM D1140 for material finer than the No. 200 sieve, and <br />ASTM C117 and C136 for coarse material) <br />• Moisture content (ASTM D2216) <br />• Atterberg limits (ASTM D4318) <br />• Standard Proctor compaction (ASTM D698) <br />• Permeability (ASTM D5084, Method D) <br />• In-place density testing (ASTM D6938) <br />• Soil description (ASTM D2488) <br />The laboratory test results are summarized in Tables 5 through 10 and individual <br />laboratory test results are presented in Appendix H.
