Cripple Creek & Victor Gold Mining Company <br />Squaw Gulch VLF Phase 1 <br />(9,550-foot Elevation Bench to Geomembrane Panel P1635) <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 2016 <br />Project No.: 74201125N0 Page 1 <br />1.0 INTRODUCTION <br />The Cripple Creek & Victor Gold Mining Company (CC&V) operates the Cresson <br />Project, located in Teller County, Colorado, near the city of Victor. CC&V was owned <br />and managed by AngloGold Ashanti (Colorado) Corp. (AngloGold Ashanti) and was <br />recently purchased in 2015 by Newmont Mining Corporation. The Cresson Project is a <br />gold mining and ore processing facility which comprises open pits, crushers, a lined <br />Valley Leach Facility (VLF), gold recovery plant, and associated infrastructure, all of <br />which were designed and constructed to meet or exceed the requirements established <br />by C.R.S. §34 32 101 et seq. and regulations promulgated there under by the Mined <br />Land Reclamation Board (MLRB). <br />The work associated with the Cresson Project is being performed under specific criteria <br />established in Amendment No. 10 of Permit Number M-1980-244, as approved by the <br />MLRB. This report documents the Construction Quality Assurance (CQA) and <br />Construction Quality Control (CQC) services provided by Amec Foster Wheeler <br />Environment & Infrastructure (Amec Foster Wheeler) from late September 2015 through <br />mid-December 2015 for the Squaw Gulch Valley Leach Facility (SGVLF) Phase 1 (9,550 <br />foot elevation bench to completed areas outlined on Figure 2), hereon known as the <br />Project. <br />This report is a continuation of the submitted Squaw Gulch VLF Phase 1 (9,450’ to 9,550’ <br />Bench) Record of Construction Report submitted November 2015. The general site <br />location is presented on Figure 1, and the Construction and Geomembrane Installation <br />Limits are presented on Figure 2 in the Figures section of this report. See Diagram 1 <br />below for an outline of areas reported in the Squaw Gulch record of construction reports.
