Review of this <br />application determined the following items must be received before the Division can consider the application as being <br />submitted and the technical review can begin. Please respond to this letter with the updated permit application pages <br />and additional information following a cover letter titled "Response to Incompleteness" and include the permit number <br />specified above. <br />APPLICATION FORM <br />f,Z \1. <br />As discussed on the phone, mineral permits held by Yuma County are all listed as Yuma County as the <br />pennittee. Please correct the question numbered 1, on page 1 of the application, where Yuma County Road <br />and Bridge Dept. was indicated to stay consistent with the other permits. Page 8 of the application, the <br />certification page, will also need to be redone and resubmitted with Yuma County as the Applicant/ Operator <br />or Company Name. <br />Within the General Operation Information section, on the first page of the application, the question numbered <br />5.1 regarding what are (if any) the incidental commoditie(s) to be mined and associated quantities per year <br />was left blank. Please correct this and re -submit Page 1 of the application. Indicate N/A if there are no <br />intended incidental commodities. <br />3. The notary on Page 8 of the application, the certification page, was incorrectly filled out. Please have the <br />notary fill in the lower portion of the page with Don Marr as Reclamation, Mining & Safety Coordinator of <br />Yuma County instead of information regarding the notary. An original will need to be resubmitted to the <br />Division. <br />EXHIBIT R— Proof of Filing with County Clerk and Recorder (Rule 6.4.18): <br />_' 4. Please note that any changes or additions to the application on file in our office must also be reflected in the <br />public review copy, which was placed with the Yuma County Clerk and Recorder by the Operator. Please <br />submit proof that the public review copy has been updated or a copy of the response to this incompleteness <br />letter has been added to it. <br />Your application will not be considered submitted until the information listed above is received and found sufficient <br />to begin our review.
