PERMIT#:M-2015-006 <br /> INSPECTOR'S INITIALS:MAC <br /> INSPECTION DATE:December 10,2015 <br /> OBSERVATIONS 1 <br /> The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br /> (Division). Matt Sprague was present on behalf of Sprague Stone LLC. The Sprague Stone site is located south <br /> of Masonville, Colorado off of County Road 27 in Larimer County.The site is perrriitted for 62.03 acres and the <br /> maximum allowed disturbed area is 16.9 acres.The approved post-mining land u4e is rangeland. <br /> The site was active at the time of the inspection.The boundary of the affected ar a was clearly delineated by <br /> monuments and fence lines.The site entrance contained a sign listing the Operat is Mine Safety and Health <br /> Administration ID number; however,the Division did not observe a permit sign a required by Rule 3.1.12.The <br /> lack of a permit sign at the site entrance has been cited as a problem and will req ire corrective action by the <br /> Operator. Please see the first page of this report for additional details regarding he corrective action. <br /> 1 <br /> As noted above,the current maximum allowed disturbed area is 16.09 acres whi4fh breaks down as follows: <br /> a) Mining areas =4.19 acres 1 <br /> b) Storage areas = 11.34 acres <br /> c) Partially reclaimed areas = 1.33 acres <br /> The Operator was conducting active mining during the time of the inspection; th* active quarry operations are <br /> located on the east side of the mine site adjacent to County Road 27. The Operator removes sandstone using a <br /> front end loader and hand tools. The Operator does not conduct blasting, but uss an expansive demolition <br /> grout to break up sandstone seams. The areas which are being quarried were previously disturbed by a prior <br /> operation at the same location.Therefore,the Operator has not commenced with topsoil salvage, but will do <br /> so as the operation moves into unaffected areas.The Operator did receive approval to import topsoil, of <br /> which approximately 100 cubic yards have been stockpiled along the eastern petmit boundary.The Operator <br /> intends to spread topsoil over the partially reclaimed areas in the spring of 2016; <br /> The Division holds a financial warranty in the amount of$38,900.00, which has been determined to be <br /> adequate to complete reclamation. <br /> This concluded the inspection. <br /> Page 2 of 4 <br />
