Inc. <br /> Permit No. M-2006-039 North Clear Creek Construction Materials Mine <br /> Garfield County, Colorado <br /> In accordance with the regulations of the Division of Reclamation, Mining and Safety, Chevron <br /> U.S.A. inc. hereby files its annual report and annual fee for the subject mine. <br /> Preceding Year <br /> It has been brought to my attention that the CDRMS has not received the 2014 Annual Report <br /> for the Chevron Clear Creek Material Mine. Please find enclosed the 2014 Annual Report, Map, <br /> Fee, and copy of the DRMS letter granting a one-time 30-day grace period effective November <br /> 19, 2015. <br /> The North Clear Creek Construction Materials Mine was opened in March of 2008 and the mine <br /> has been in operation since that time. No material was removed from the mine FY 2014 and no <br /> new reclamation or revegetation activities were initiated. A noxious weed control program has <br /> been in place since the inception of the mine and the site is regularly monitored for noxious <br /> weeds. No weed treatments were necessary or conducted at the mine in 2014. Existing <br /> stormwater management controls are monitored at regular intervals and maintained as needed. <br /> The Mine is currently in Intermittent Operation status as approved by CDRMS in November of <br /> 2012. <br /> A check in the amount of $323.00 for the annual fee, along with a survey map of the current <br /> mine showing the disturbed areas are enclosed. <br />
