The report was received via e-mail on October 28 th, 2015. The report documents the sealing of
eight wells that were used in a US Geological Survey study that was conducted within the permit area.
It does not appear these wells have any permitted monitoring requirements. Each well was reported as an
artesian well and was sealed with cement that was tremied into the wells. The report indicates the wells
were sealed on September 24, 2015. Included with the report was the US Geological Survey Open-File
92-122 which is the report/study where these wells were utilized. The US Geological Survey report
includes details on the location of the drill holes and a map depicting the holes, the depth of the holes and
surface elevation of the holes. The abandonment report includes the diameter of the drill holes, the type
of cement used as well as the volume of the cement used to seal the drill holes. The abandonment report
submitted includes the name of the drilling contractor but the license number of the drill and rig was not
reported as required by Rule 4.07.3(3)(h). Please provide this information.
If you have any questions feel free to contact me at (303) 866-3567, extension 8120.
Sincerely,

Jared Ebert
Environmental Protection Specialist
1313 Sherman Street, Room 215, Denver, CO 80203P 303.866.3567 F 303.832.8106 http://mining.state.co.us

John W.
