PERMIT #: M-1985-140 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 18, 2015 <br />OBSERVATIONS <br />This inspection was conducted as part of the regular monitoring program. The Operator was notified of the <br />inspection by telephone, but was unable to be present for the inspection. This is a 110c gravel mine. It was <br />not operating at the time of the inspection. <br />The Goss Sand & Gravel Pit entrance is located approximately 12 miles east of Falcon on the north side of <br />Falcon Hwy (about 0.5 miles east of N. Ellicott Hwy). <br />Inspection: <br />A permit sign was displayed at the site entrance (see Photo 1). The paint on the sign is beginning to peel and <br />may become illegible in the near future. The east (see Photo 2) and south permit boundaries are fenced. As <br />the Operator was not present, it is unclear how or if the west and north permit boundaries are marked. <br />This inspector walked the pit area (see Photos 3 and 4). Disturbed areas (pit, haul road and truck turnaround <br />area) were measured with a Trimble Juno 3B handheld GPS unit (see Figure 1). The fresh heavy equipment <br />tracks and highwall excavations (see Photo 5) suggested recent mining activity. The highwalls were estimated <br />to be between 8 and 10 feet high with recent excavations at near vertical. A possible topsoil stockpile was <br />observed on the north side of the pit (see Photo 6). No noxious weeds were observed. <br />Records: <br />• The total permitted area is 9.9 acres. Figure 2 shows the permit boundary estimated from dimensions <br />provided in the original permit application. <br />• The previous inspection was performed on 10/13/2010. No problems were cited. <br />• The anniversary date is January 24. Annual reports and fees are current through 2015. However, the <br />annual reports for at least the last five years have been incomplete in that not all the information <br />requested in these reports have been provided. <br />• Revisions: Technical Revision (TR -01) was approved by the Division on 4/1/2014 designating this <br />operation as being in intermittent status. As such mining activity is required to occur at least once <br />every year. <br />• The post -mine land use is developed water resources. <br />Current Stipulations: <br />None. <br />Bond: <br />There is a $5,000 financial warranty (cash) for the site. Based on GPS measurements and assuming the 770 - <br />foot long haul road is no more than 10 feet wide, the current disturbed area estimate is 0.71 acres (0.43 ac + <br />0.10 ac + 770'x10'/43,560 sqft/ac). The current bond is deemed adequate for the current disturbance. <br />Summary and Recommendations: <br />1. The permit sign should be replaced or repainted soon to remain legible. <br />2. The Operator is required to provide all the information requested in the annual reports. Please <br />completely fill out future annual reports. <br />3.
