PERMIT#:M-1981-185 <br /> INSPECTOR'S INITIALS:DMC <br /> INSPECTION DATE:October 22,2015 <br /> OBSERVATIONS <br /> This inspection was conducted by Dustin Czapla, Russ Means, and Lucas West of the Division of Reclamation,Mining <br /> and Safety.The operator was notified of the inspection on October 19, 2015,but a representative for the operator was not <br /> present. Photographs are included with this report in order to illustrate some of the conditions observed. <br /> The May Day Idaho Mine Complex is located approximately twelve miles northwest of Durango and accessed from CR <br /> 124.The site is a I l2d-1 operation that includes a total of 274.7 permitted acres. The Division currently holds a financial <br /> warranty amount of$204,562.00 for this site.This amount is considered adequate at this time. <br /> A mine identification sign was not posted at the entrance to the site.This was noted as a problem for failure to comply <br /> with Rule 3.1.12(1). <br /> The operator has failed to control noxious weeds. Common mullein and bull thistle were observed along the roads <br /> throughout the site.This was noted as a problem for failure to comply with Rule 3.1.10(6). <br /> During the June 5, 2013 inspection it was noted that the sand bags utilized as part of the stormwater sediment detention <br /> structures were deteriorated and leaking sand down the slopes where they were placed.This was noted as a problem in the <br /> June 2013 report, with a corrective action deadline of July 25,2013.There was no evidence observed during this <br /> inspection that the operator has made any attempt to resolve this problem. <br /> New Access Road <br /> The new access road has been completed.The cut and fill slopes along the road appear stable and are becoming well <br /> vegetated.The Redi-Rock retaining wall appears stable. No signs of movement were noted. Guard rails are in place along <br /> the road in order to address MSHA requirements.The road surface appears stable and concerns regarding erosion were <br /> not noted. A boulder has been placed on top of the retention pond drain structure, which was apparently lifted during a <br /> heavy rain event. At this time, the boulder appears to be effectively holding the drain structure down. A more permanent <br /> anchoring system may be necessary in the future. Surplus Redi-Rock has been placed at the north end of the road,just <br /> before the bridge. Sixteen blocks were noted. <br /> Renovation of the La Plata River bridge,as approved through TR-3, has not yet commenced. <br /> T <br /> ?. w.1. <br /> 3 +4Cst ��.
