Sample Shipment <br />The samples are shipped in compliance with DOT regulations. Samples are shipped in coolers <br />with ice. The shipment must include a signed Chain of Custody Form. <br />B.4.5. Sample Receipt <br />Upon sample receipt, the contracted analytical laboratory is required to: <br />• Sign, date, and note the time on the Chain of Custody Form, indicating sample receipt <br />• Assign unique laboratory identification numbers to the samples <br />• Contact the laboratory coordinator to resolve any discrepancies in documentation or <br />samples received <br />• Return a copy of the Chain of Custody Form, if applicable, to the laboratory coordinator <br />• The laboratory shall notify the laboratory coordinator within 24 hours of discovery of lost, <br />damaged, or destroyed samples <br />• The laboratory shall provide a Sample Condition Upon Receipt report to the laboratory <br />coordinator within 24 hours of receipt, as applicable <br />It is the responsibility of the laboratory coordinator to ensure that the laboratory complies with <br />these requirements. <br />B.4.6. Analytical Reports <br />The laboratory sends an analytical report to the laboratory coordinator at the completion of <br />sample analysis. The laboratory coordinator then initiates data review and validation. <br />B.4.7. Records <br />Records generated during the sample submittal process are kept on file electronically by NS, <br />ACZ, and Daub. They are identifiable by the Project ID assigned to them by ACZ. <br />B.S. Standard Practice for Decontamination and Freeze Protection of Field <br />Equipment <br />B.5.1.
