PERMIT #: M-1981-185 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: October 22, 2015 <br />OBSERvA'HONS <br />This inspection was conducted by Dustin Czapla, Russ Means, and Lucas West of the Division of Reclamation, Mining <br />and Safety. The operator was notified of the inspection o„ Oetobe, 19, 2015; but a ,ep,e6entative for the operator was not <br />present. Photographs are incluued with this report in order to illustrate some of the conditions observed. <br />The May Day Idaho Iviine Complex is located approximately twelve miles northwest of Durango and accessed from CR <br />124. The site is a 112d-1 operation that includes a total of 274.7 pe„n;tted ae,e5. The Divi3►o„ Cu„e„ tly holds a financial <br />warranty amount of $204,5b2.00 for this site. This amount is considered adequate at this time. <br />A n,;,,e identification gig„ waa not po6ted at the ent,ance to the site. This was noted as a problem for tailure to comply <br />With Rule 3.1.12 (1). <br />The operator has failed to control noxious weeds. Common mullein and bull thistle were observed along the roads <br />throughout the 5it'c. This waa noted an a p,oblCi„ fo, failuie to comply with Rule 3.Llu (b). <br />During the June 5, 2013 inspection it was noted that the said bags utilized aa pan of the atC„nwate, 6edi,,,ent detention <br />structures were deteriorated and leaking sand down the slopes where they were placed. This was noted as a problem in the <br />Ju„e 2013 ,epurt_ with a coueeG vu aetlon deadlinu of July 25, 2013. t here was no evidence observed during this <br />inspection that the operator has made any attempt to resolve this problem. <br />New Acceaa Riad <br />The new access road has been completed. The cut and fill slopes alo„g the ,oad appea, stable and a,e becon,;,,g well <br />vegetated. The Redi-Rock retaining wall appears stable. No signs of movement were noted. Guard rails are in place along <br />the road in order to add.u33 MSHA ,eq.nement5. The ,vad 3urfacr appears stable and concerns regarding erosion were <br />not noted. A boulder has been placed on top of the retention pond drain structure, which was apparently lifted dt„i„g a <br />heavy ,a►„ event. At thin. ti,,,e, the boulder appears to be effectively holding the drain structure down. A more permanent <br />anchoring system may be necessary in the future. Surplus Redi-Rock has been placed at the north end of the ►Cad; just <br />before the bridge.
