(RGS) for <br /> $1,150.00 as payment for the annual fee. <br /> Changes to Permit <br /> There was a in was granted <br /> JIova or Sampling Frequency Reduction of Surface and Groundwater Monitoring. <br /> S7ampling Frequency Reduction of Surface and Groundwater Monitoring <br /> ities continue to be reduced at the Bulldog Project in 2015 due t market <br /> ci o cu costs, RGS requested a reduction in sampling <br /> frequency requirements required by the Environmental Protection Plan. RGS requested <br /> that the frequency in Windy Gulch be reduced from four times per year (quarterly) to <br /> one time per year. The request was approved on September 16, 2015. The annual <br /> sampling in 2016 will be conducted in the spring for surface water and in the fall for <br /> groundwater. <br /> Amendment AM-2 to Permit No. M1977-215 <br /> The Division approved Amendment AM-2 on December 15, 2014. The initial <br /> Environmental Protection Plan (EPP) for the project addresses the four areas listed <br /> below: <br /> • Waste rock placement on the surface <br /> A detailed waste Rock characterization study was conducted and a final <br /> summary was submitted to the Division in May 2015. <br /> • Stormwater controls <br /> A Report on the Adequacy of Storm Water Runoff Structures was completed in <br /> April 2014 and shows the structures to be adequate. <br /> • Surface Water and Groundwater monitoring plans <br /> Quarterly surface and groundwater analytical results in 2014 and 2015 have <br /> been submitted to the Division through 2Q15. 3Q15 analytical results are <br /> included in this annual report, see Figure 3 for groundwater and Figure 4 for <br /> surface water. 2016 analytical results will be submitted in accordance with the <br /> approved sampling frequency reduction. <br /> • Emergency Response Plan <br /> The Emergency Response Plan is current. <br /> 625 USFS Road 504.1A P.O.
