PERMIT #: M-1977-534 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: October 20, 2015 <br />CORRECTIVE ACTIONS: The operator shall submit a Technical Revision, with the required $216 revision fee, to <br />update and clarify the current approved reclamation plan to reflect existing and proposed activities by the <br />corrective action date. The Technical Revision shall specify the seed mix which was used and shall specify the <br />amount of the topsoil imported to the site as well as the source of the topsoil. In addition, the Operator shall <br />submit an affidavit certifying that the imported topsoil was clean and inert as required by Rule 3.1.5(9).* <br />CORRECTIVE ACTION DUE DATE: 11/30/15 <br />INSPECTION TOPIC: Sediment Control <br />PROBLEM/POSSIBLE VIOLATION: Problem: Erosion gullies and ruts were observed on-site. This is a problem at <br />this time for failure to protect the affected land from erosion pursuant to C.R.S. 34-32.5-116 (4) (j). <br />CORRECTIVE ACTIONS: The operator shall provide photo documentation to the Division verifying erosion gullies <br />and ruts have been repaired, and that the site has have been reconstructed and stabilized to prevent erosion <br />damage by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 11/30/15 <br />*Several of the above cited Problems require the Operator to submit a Technical Revision. All of the required <br />changes to the Reclamation Plan may be submitted under a single Technical Revision, with a single fee of <br />$216. <br />Page 2 of 7 <br />
