Grandview Dr. <br />Tempe, AZ 85281 <br />October 20, 2015 <br />Division of Reclamation, Mining, and Safety <br />1313 Sherman St., Room 215 <br />Denver, Colorado 80203 <br />Annual Report: Deep Creek Placer (Permit No. M-1986-102) <br />Dear Sir or Madam: Please accept this Annual Report on the "Deep Creek Placer," an inactive <br />limestone mine 7 miles west of Telluride, Colorado at the confluence of Deep Creek and the San <br />Miguel river. <br />--No mining has taken place in the year Sept. 5, 2014 — present <br />--Portals to the mine have been checked and they are secure <br />--Equipment room entrance locked and secured. <br />--Routine maintenance of access road carried out, summer/fall 2015 <br />--Annual maintenance of storm water "ponds" along access road for capturing runoff is complete. <br />--Access road was cleared and graded. <br />--Large boulder blocking storm water run-off was drilled and split using non -explosive <br />demolition agent, DexPan (8/15). <br />--New culvert installed (11/14) at foot of access road at intersection of Mine Road and CR639. <br />Modifications will be required by San Miguel County converting the full culvert to a V -Pan type <br />installation to ensure unobstructed flow. <br />--Access road gate is secure and locked <br />--No disturbances to the surrounding land or the mine itself are anticipated. <br />--Regular maintenance of roads, equipment apron, portals, and storm water ponds will be <br />conducted by myself and caretaker Anton Viditz-Ward over the course of the coming year. <br />Other factors to report: <br />1. Inactive Mine Status <br />While there are on-going discussions in the family regarding future uses of the mine, the <br />cost of fuel and other factors such as the demand for limestone in the marketplace make it <br />difficult to predict what commercially viable opportunities will present themselves in the <br />near future. Still, as there are very few commercially accessible limestone sources within <br />a 100 mile radius of the mine, we feel the mine has future potential as a source for high <br />quality calcium carbonate. <br />2. Succession of Operator <br />We have successfully completed the Succession of Operator process. The outgoing <br />operator was B -Mining of Nucla, CO (contact: Michael Moore, CEO, B -Mining). The <br />new operator is Collins -Morehead LLC (Daniel L. Collins, managing partner, Collins - <br />Morehead, LLC). <br />3. Revised Reclamation Plan <br />1 <br />
