PERMIT #: M-2010-034 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: October 1, 2015 <br />acres, which is 2.63 acres greater than the approved 8.75 acres. The Division estimates 0.54 acres have been <br />disturbed outside of the marked boundary. As cited as a problem, the Operator must conduct a survey and <br />replace the boundary markers in the correct location(s). The Operator shall provide proof to the Division this <br />has been done by the corrective action date. The Operator will be required to submit a conversion application <br />to the permit to include all mining disturbances outside of the 9.99 acres permit boundary if the Operator's <br />survey indicates area outside of the permitted 9.99 acre site have been affected by mining activity. A copy of <br />the Division's survey map is attached for reference. <br />A scale house area has been created by the Operator. The scale house location is not approved under the <br />current mine plan. The Division estimates 0.51 acres have been disturbed outside of the permit boundary. As <br />cited as a problem, the Operator must provide an explanation for the new disturbance by the corrective action <br />date. The Operator will be required to submit a conversion application to the permit to include scale house if <br />the disturbance is mining related. The location of the scale house disturbance is indicated on the attached <br />inspection map. <br />The approved Mine Plan states the mine will be sloped so all surface water runoff can be contained on the <br />site. Two erosion gullies were observed during the inspection. The erosion gullies were located south of the <br />entrance road in the northeast corner of the site and along the eastern boundary of the site. The location of <br />the gullies are indicated on the attached inspection map. As cited as a problem, the Operator shall provide <br />photo documentation to the Division verifying the erosion gullies have been repaired, and the site has been <br />reconstructed and stabilized to prevent erosion damage by the corrective action date. <br />The site was active during the inspection. A processing plant was located in the north end of the site. Active <br />excavation and reprocessing of the stockpiled material along the northern permit boundary were occurring. <br />Recent topsoil stripping and stockpiling in the northwest corner of the site was observed. Topsoil stockpiles <br />were observed along the perimeter of the site as noted in the previous inspection report. No exposed <br />groundwater or standing stormwater was observed during the inspection. Evidence of noxious weed spraying <br />was observed. The mine identification sign was posted as required by Rule 3.1.12(1). Photographs taken <br />during the inspection are attached. <br />Page 3 of 8 <br />
