Compliance With Mine Plan <br />PROBLEM/POSSIBLE VIOLATION: Problem: The affected area boundary markers are incorrectly placed based on <br />a GPS survey by the Division. This is a problem for failure to maintain boundary markers around the affected <br />area as required by Rule 3.1.12(2). <br />CORRECTIVE ACTIONS: The Operator must conduct a survey and replace the boundary markers in the correct <br />location(s). The Operator shall provide proof to the Division this has been done by the corrective action date. <br />The Operator will be required to submit a conversion application to the permit to include all mining <br />disturbances outside of the 9.99 acres permit boundary if the Operator's survey indicates area outside of the <br />permitted 9.99 acre site have been affected by mining activity. <br />CORRECTIVE ACTION DUE DATE: 11/19/15 <br />INSPECTION TOPIC: Sediment Control <br />PROBLEM/POSSIBLE VIOLATION: Problem: Erosion gullies were observed on-site. This is a problem for failure <br />to protect the affected land from erosion pursuant to C.R.S. 34-32.5-116 (4)0). <br />CORRECTIVE ACTIONS: The Operator shall provide photo documentation to the Division verifying the erosion <br />gullies have been repaired, and the site has been reconstructed and stabilized to prevent erosion damage by the <br />corrective action date. <br />CORRECTIVE ACTION DUE DATE: 11/19/15 <br />Pagel of 8 <br />
