PERMIT #: M-2001-087 <br />INSPECTOR'S INITIALS: TCI <br />INSPECTION DATE: August 6, 2014 <br />OBSERVATIONS <br />This inspection was conducted as part of the regular monitoring program. The Operator (Fremont County) was <br />represented by Mr. Sterling Rife who was present for the inspection. <br />The Indian Springs Park Pit site access road entrance is located approximately 13 miles south of Hwy 50 and Co. <br />Rd 3 (Copper Gulch Rd) off Fremont co. Rd 28. This is a 112c gravel mine. The mine was not operating at the <br />time of the inspection. <br />Inspection: <br />Markers & Boundary —A permit sign was displayed at the site entrance (see Photo 1). The Operator has marked <br />the permit boundary with T -posts (see Photo 2). <br />Mining Plan — Highwalls were observed to be steeper than 3H:1V (see Photo 3), but appeared stable. Mr. Rife <br />explained the material is rippable and that blasting is not currently necessary. This inspector reminded Mr. Rife <br />that the permit has a stipulation requiring a geotechnical stability exhibit be submitted and approved by the <br />Division prior to initiating any blasting activity. Several material stockpiles were also observed on the pit floor <br />(see Photo 4). <br />Reclamation — No reclamation had been initiated. Topsoil stockpiles were placed near the permit boundary and <br />labeled in typical Fremont Co. manner (see Photo 5). Backfill material for reclamation was stockpiled offset 15 <br />to 20 feet from the crest of the highwall (see Photo 6). <br />Environmental — No problems with contaminant spills, erosion/sedimentation or noxious weeds were observed. <br />Some standing water was observed in the deeper east pit. Mr. Rife explained that water was runoff from recent <br />heavy rains (see Photo 7). <br />Records: <br />• Revision: no revisions to date. <br />• Commitments/Stipulations: Stipulation #1 has been complied with. Stipulation #2: Prior tc <br />conducting any blasting at the Indian Springs Park Pit, the operator will submit and receive <br />approval for a Geotechnical Stability Exhibit in accordance with Rule 6.5(4) of the Construction <br />Materials Rules and Regulations of the Colorado Mined Land Reclamation Board. <br />• Other Records <br />a) The total permitted area is 20 acres, with a maximum allowed disturbed area (at one time) <br />of 20 acres. <br />b) The previous inspection was performed on 10/30/2006. No problems were cited. <br />c) The anniversary date is February 11. Annual reports and fees are current through 2015. <br />d) The post -mine land use is rangeland. <br />Bond: <br />As this is a county pit, no bond is required. <br />Summary and Recommendations: <br />1. No problems or concerns were observed during the inspection. <br />2. The Operator is reminded that should blasting be required in the future at this pit, Stipulation #2 requires <br />the submittal of a Geotechnical Stability Exhibit in accordance with Rule 6.5(4) of the Construction <br />Materials Rules and Regulations of the Colorado Mined Land Reclamation Board. <br />Please contact Tim Cazier (303-866-3567, ext. 8169) if you have any questions regarding this report. <br />Page 2 of 6 <br />
