Phone 719-288-1402 <br />3500 State Hwy 120 Fax: 719-784-3470 <br />Florence, CO 81226 www.holcim.com/us <br />Re: Holcim (US) Inc., Boettcher Limestone Quarry, Permit M-1977-348 <br />Dear Mr. Dupler, <br />Attached please find a check in the amount of $791.00 for the Limestone Boettcher Quarry to <br />cover the annual fee for the period from October 10, 2014 to October 10, 2015. <br />The Limestone Boettcher has been in a status of final reclamation. Final reclamation has been <br />completed and is pending release due to some water issues at the site. Activities on the site <br />have been as follows: <br />Weed control in coordination with Larimer County Weed Department — Activity Report for <br />2014 is attached and the program is continuing in 2015. <br />2. Monitoring and sampling the water wells on the property. <br />3.
