The spill of P2S5 occurred during the mixing <br />of Nokes, a chemical reagent used as a depressant in the molybdenum ore flotation process, which is a <br />mixture of P2S5, sodium hydroxide (NaOH) and water. The initial incident, while short in duration, had <br />longer -lasting implications, which drew the entire response out through Friday of the same week. At <br />present, all cleanup has been performed and a comprehensive root cause analysis is in the process of <br />being performed. <br />(a) The Nokes mixing process was immediately and automatically stopped via digital control <br />system programming interlocks, and emergency response personnel were mobilized to <br />contain and clean up the spill of P2S5. <br />(b) The release of hydrogen sulfide on September 22-25 from this incident did not adversely <br />impact human health, property or the environment. <br />(c) The contact person for this event is: <br />Tim Haynes <br />19302 County Road 3 <br />Parshall, CO 80468 <br />729-942-3518 <br />timothy haynes(ofmi.com <br />(d) No analyses were completed as a result of this event. Monitoring of hydrogen sulfide <br />levels in the immediate area of the Nokes building were generally well below <br />occupational action levels. Monitoring levels at the nearest point of public contact, <br />County Road 3, were non-detectable. <br />
