Box 187 <br />Craig, CO 81626 <br />RE: Trapper Mining, Inc., Permit C-1981-010 <br />Permit Revision No. 7, AVS Check Results <br />Dear Mr. Luke: <br />The Colorado Division of Reclamation, Mining and Safety is required to conduct a final Applicant <br />Violator System (AVS) check prior to issuing the final decision for permit revisions. The end of the <br />public comment period for Permit Revision No. 7 (PR7) is October 5t'', 2015. Given this, the Division <br />reviewed the Ownership and Control Information contained in section 1.2.1 of the permit against the <br />information in the AVS. One item was found that will need to be addressed prior to the issuance of the <br />final decision for PR7. <br />1. Revised page 1-7b includes a list of the Directors for Tri-State. It appears that Gary Merrifield, <br />William Patterson, and Gerald Seward are no longer Directors for Tri-State; all have end dates <br />per the AVS system. These individuals are listed on revised page 1-7b as Directors. The AVS <br />system indicates that Don Schutz, John Gavan, and Don Kaufman have taken their place. Please <br />submit a minor revision and update the Tri-State Directors list accordingly. <br />The Division will not be able to issue the final decision for PR7 until the minor revision is approved <br />updating the ownership and control information.
