July 23, 2015 C-1981-035/King Coal Mine RAR <br />At the King II sediment pond the top lip of the outlet pipe for the primary spillway was slightly <br />warped, and should be straightened. <br />INSPECTION. - <br />SIGNS AND MARKERS Rule 4.02 <br />o Mine Identification signs Permit Boundary markers and surface disturbance markers identified <br />during this inspection were installed as required throughout the areas visited. <br />o Topsoil stockpiles observed were clearly marked with identifying signs. <br />ROADS Rule 4.03 <br />Construction 4.03.1(3) 14.03.2(3); Drainage 4.03.1(4) 14.03.2(4); Surfacing and Maintenance <br />4.03.1(5) and (6) 14.03.2(5) and (6); Reclamation 4.03.1(7) 14.03.2(7): <br />o All roads in and around the facilities area were stable. The main haul road at King II was in good <br />condition. No dust was being generated. <br />o East Clearwater Ditch along the main haul road changes gradient near the mine entrance. The <br />ditch is stable with no soil moving and no evidence of erosion (Photo 1). There is no need to add <br />riprap or other stabilizing material at this knickpoint. <br />SUPPORT FACILITIES- Rule 4.04: <br />o Volumes of coal at both stack tubes (91 and 92) were contained in the designated areas. <br />Flags were visible and easy to identify. <br />o There were a number of haul trucks waiting to be loaded with coal and some trucks were <br />loaded during the inspection. <br />HYDROLOGIC BALANCE- Rule 4.05 <br />Drainage Control4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6,- Discharge <br />Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />o The King II sediment pond held water and was not discharging. The primary and emergency <br />spillways were clear (Photo 1). <br />o The West Clear Water Ditch was free of debris, adequately rip rapped and appeared to <br />be functioning correctly. The ditch was not transmitting water at the time of inspection. <br />After an aerial survey in November of 2014, the ditch had been surveyed by a <br />professional surveyor in March 2015 to assure it met design criteria. <br />o There is an upgradient NPDS monitoring point at the Ute Mountain Tribal Building in <br />Hay Gulch as well as a NPDS monitoring point at the discharge to the sediment pond. <br />o The East Clear Water Ditch passes through a reclaimed area. This disturbed area was <br />voluntarily reclaimed by the King Coal mine and comprises diverse established <br />Number of Partial Inspection this Fiscal Year: 1 <br />Number of Complete Inspections this Fiscal Year: 0 <br />Page 3 of 9 <br />
