The decision
date was extended to October 30, 2015 to allow sufficient review and response time. Please be
advised that if you are unable to satisfactorily address any concerns identified in this review before
the decision date, it will be your responsibility to request an extension of the review period. If
there are outstanding issues that have not been adequately addressed prior to the end of the review
period, and no extension has been requested, the Division will deny this technical revision.

The following comments are based on the Division’s review of the Request for Technical Revision
TR-03:

6.5 GEOTECHNICAL STABILITY EXHIBIT
1. It is not clear whether one or two geologic cross sections were evaluated. The “Highwall
Survey Map” prepared by Azurite Inc. (plan view with blue contour lines and a satellite
image as a background) only shows Section A-A. This section appears to be shown on
the following page of the submittal, labeled “Cross Section Aʹ-A, Red Canyon Quarry”.
However, the page following Cross Section Aʹ-A, depicts an approximately 200-ft high
section labeled “Final Bench Configuration, East Side, Looking Northeast”. There is no
horizontal reference line for this section on the aforementioned “Highwall Survey Map”.
Further, when referencing the Reclamation Plan Map from Technical Revision No. 2,
received March 11, 2013 the south highwall on the east side of the pit reaches a height of
just over 260 feet. Please clarify:
a. How many sections are analyzed,
