PERMIT #: M-1985-022 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: September 18, 2015 <br />ponds. This type of change to the Mining Plan can be addressed through a Technical Revision. The Operator <br />should note that this type of change would also need to be reflected on the Mining Plan Map. <br />Signs and Markers: <br />The Division noted that there was no sign posted at the site entrance which identified the name of the <br />Operator or the permit number for the operation. The Operator should post the permit sign at the site <br />entrance where the locking gate is located. <br />Pursuant to Rule 3.1.12, the boundaries of the affected area are to be marked with monuments or other <br />markers that are clearly visible and adequate to delineate such boundaries. The Operator has marked the <br />north and northeastern portion of the permit boundary where active mining operations are taking place. <br />However, the southern portion of the permitted area where the sediment ponds are located did not contain <br />boundary markers. <br />The lack of a posted permit sign and boundary markers have been cited as problems and will require <br />corrective action by the Operator. Please see the first page of this report for additional information. <br />This concluded the inspection. <br />(The Operator should note that multiple changes to the Mining and Reclamation Plan can be included under a <br />single Technical Revision submittal and the fee of $175.00 only need to be submitted once for each Technical <br />Revision.) <br />Page 5 of 7 <br />
